About this resource

This program provides at-home support that encompasses services such as housekeeping, personal assistance, laundry, shopping, transportation, and meal preparation and delivery. It also includes adult day services, which are non-residential community programs available for part of the day, and registered nurse services that may involve one-time or periodic assessments, service planning, medication oversight, provider training, and educational support. Additionally, case management and options counseling are offered to aid individuals in making informed choices about services tailored to their strengths, needs, and preferences. The number of service hours allotted to each person is determined by their assessed needs and available funding, typically ranging from 5 to 20 hours monthly. Importantly, benefits received will not be subject to estate recovery.

Who can use this

  • Adults aged 18 and over who require personal care assistance, have an income not exceeding 400% of the Federal Poverty Level, possess assets below the average cost of six months in a nursing facility, and reside in their own home.
