About this resource
This program delivers confidential case management and independent living support tailored for individuals diagnosed with HIV. It offers financial aid for HIV medications, transportation to medical appointments, case management sessions, insurance premiums, transitional housing, rental assistance, dental services, disease management education, and referrals to other agencies for essential needs, employment, and substance abuse treatment.
Intake Process: Schedule an appointment via phone.
