This program provides various support services, including case management, personalized support plans for individuals and families, transportation brokerage, in-home assistance or residential options, and vocational services.
Intake Process: To begin, call to obtain an application for Developmental Disabilities services. An eligibility worker will arrange an appointment to guide you through the application process.
Costs: There are no fees associated.

- Eligibility is for individuals with an intellectual or developmental disability who are residents of Oregon and US citizens.
Documents you may need
- Proof of citizenship, proof of address, and records for all psychological and medical assessments related to the disability
