About this resource
Patients receive individualized support from a Certified Community Health Worker and a Registered Nurse throughout their pregnancy journey. Services include prenatal and postpartum care, education on pregnancy, and assistance with breastfeeding and breast pump usage. Additionally, mental health support and family planning services are available.
To initiate services, please call to schedule an appointment.
Payment Information: The program accepts OHP, Medicare, and private insurance, and provides a sliding scale discount for those who are uninsured.
