Delivers a variety of healthcare services such as primary care, dental, optometry, specialized medical care, WIC, nutrition guidance, maternal and child health services, and a diabetes program. A mobile medical bus is also available solely on the Warm Springs Reservation.

- Eligibility is restricted to members of federally recognized American Indian or Alaskan Native tribes or their children.
