About this resource
This program provides a variety of support services tailored for individuals facing intellectual and/or developmental disabilities. These services encompass case management, personalized support plans for individuals and families, access to brokerage services, in-home assistance or residential options, vocational training, and protective services in cases of abuse.
Intake Procedure: To initiate the process, call to obtain an application for Developmental Disabilities services. An eligibility worker will arrange an appointment to guide you through the application steps.
Travel/Location: Located adjacent to the Hawthorn Farm MAX stop.
Fees: There are no fees associated with these services.
Who can use this
- Individuals with an intellectual or developmental disability who are Oregon residents and U.S. citizens.
