About this resource
This program provides a range of support services for individuals with intellectual and/or developmental disabilities. These services encompass case management, support plans for individuals and families, access to brokerage services, in-home assistance, residential placement, as well as investigations into abuse and protective services.
Intake Procedure: To initiate the process, call to request an application, after which an appointment will be arranged to assist with the application.
Travel/Location: Located adjacent to the Hawthorn Farm TriMet MAX stop.
Fees: There are no fees associated with these services.
