Travel/Location: The facility is situated off 99W and SW Durham Road in the Willowbrook Center, close to King City, and is accessible via TriMet bus line 12.
Fees: Charges range from $40 to $45 per hour, $150 for a full day, with monthly rates available for regular participants. Payment options include cash, checks, ACH, and major credit cards such as Visa, MasterCard, and Discover.
Who can use this
- Medicaid eligibility starts at age 55; private pay is available for individuals aged 18 and older.

- Online enrollment form, medication list if applicable, Portable (or Physician) Orders for Life-Sustaining Treatment (POLST) if applicable, and/or Advanced Directive form if applicable
