This initiative provides a variety of services tailored for older adults and individuals with disabilities, which encompass assessments, case management, and the management of SNAP (food stamps), Oregon Supplemental Income Program (OSIP), along with other Medicaid programs, Medicare Savings Programs, and guidance for accessing coordinated services.
