This service offers eligibility assessments for Medicaid and SNAP, licenses adult foster care facilities, conducts both private and pre-admission nursing home evaluations, provides Senior Peer Mentoring, and promotes various health and wellness initiatives.

Travel/Location: When approaching McMinnville via Highway 99, take a right (west) onto Second Street by the McMinnville Public Library. Proceed for four miles until you reach a business complex on the left. Turn left at the first entrance into this complex, navigate between two buildings, and cross over a speed bump. The next building on the left will be a one-story beige structure.

- Individuals aged 60 and above, as well as those with physical disabilities aged 18 and over.
