About this resource
This program offers a variety of support services tailored for individuals with intellectual and/or developmental disabilities. Available services encompass case management, personalized support plans for individuals and families, access to transportation brokerage services, in-home assistance or residential options, as well as employment and vocational training.
Intake Procedure: To begin, call to obtain an application for Developmental Disabilities services. An appointment will be arranged with an eligibility worker who will guide you through the application process.
Fees: There are no fees associated with these services.
Who can use this
- Individuals with an intellectual or developmental disability who are Oregon residents and US citizens.
Documents you may need
- Proof of citizenship, proof of address, and records for all psychological and medical assessments related to the disability
