Nutrition Support is a service under the Oregon Health Plan that addresses health-related social needs (HRSN). It assists members with health conditions where nutritional support can enhance health outcomes. This service may encompass: Medically Tailored Meals (MTMs), which include fresh or frozen meals delivered to the member's home for up to six months, with a maximum of three meals per day. Additionally, Nutrition Education provides evidence-based methods for making healthier food choices and culturally appropriate dietary guidance. Pantry stocking offers a portion of recommended daily food for up to six months, available as a grocery box or a pre-loaded card for specific grocery stores. Members can also receive benefits for fresh, canned, frozen, or dried fruits and vegetables for up to six months, delivered similarly. The intake process involves contacting the service via phone, email, or website, followed by a referral to a registered dietitian for assessment to validate the need for MTMs and ensure care plan objectives are met. There are no associated fees.

Who can use this

  • Members of AllCare facing food insecurity can access HRSN nutrition services only once in their lifetime, with restrictions on simultaneous MTMs and Nutrition Education. Eligibility requires experiencing specific life situations and health conditions.
