This program provides a FREE wireless phone and offers UNLIMITED data, text, and minutes monthly for those who qualify. Eligibility includes recipients of Medicaid, Supplemental Nutrition Assistance Program (Food Stamps or SNAP), Supplemental Security Income, Federal Public Housing Assistance, and Veterans and Survivors Pension Benefit.
Application can be submitted via mail or fax with a completed form, and there is also an option to apply online.
Fees are based on a sliding scale.
