Nutrition Support is an HRSN service offered by the Oregon Health Plan aimed at assisting members with health-related issues that can be improved through nutritional support. This service may encompass: Medically Tailored Meals (MTMs), which include fresh or frozen meals delivered to the member's home for up to six months, allowing for up to three meals a day. Nutrition Education, which provides evidence-based guidance on healthy food choices and culturally appropriate dietary practices. Pantry stocking, which supplies a portion of the recommended daily food intake for up to six months, available as a grocery box or pre-loaded card for certain stores. Additionally, fresh fruits and vegetables are provided for six months, also delivered as a grocery box or through a pre-loaded card. The intake process involves contacting the service via call, email, fax, or website, followed by a referral to a registered dietitian for assessment. There are no fees associated with this service.
