About this resource

Nutrition Support is a service under the health-related social needs (HRSN) umbrella provided by the Oregon Health Plan, designed to aid members facing health-related issues that could benefit from nutritional interventions. This service encompasses several components: Medically Tailored Meals (MTMs), which may include fresh or frozen options, providing up to three meals daily for a maximum of six months, delivered to the member's home or residence, including shelters without meal provisions. Nutrition Education offers evidence-based strategies for improving food choices and nutrition-related behaviors, focusing on personalized, culturally appropriate dietary guidance. Pantry stocking supplies a portion of the recommended daily food intake for up to six months, with options for fresh, frozen, dried, pureed, or canned foods, delivered as a grocery box or via a pre-loaded card for select grocery stores. Additionally, a benefit for fruits and vegetables is available for six months, including various forms of produce, also delivered or accessible through a pre-loaded card.

The intake process involves calling, emailing, or visiting the website for further details. Following an initial screening, a referral is made by the CCO to a registered dietitian for an assessment to confirm the necessity for MTMs and to ensure that the goals of the MTM care plan are met.

There are no fees associated with this service.

Who can use this

  • Members of Cascade Health Alliance facing food insecurity can access HRSN nutrition services once in their lifetime, with restrictions on simultaneous receipt of MTMs and Nutrition Education. Eligibility requires experiencing specific life situations and health conditions.
