About this resource
Nutrition Support is a health-related social need (HRSN) service provided by the Oregon Health Plan aimed at assisting members with health-related conditions that can benefit from nutritional support. This service may include: Medically Tailored Meals (MTMs), which can consist of fresh or frozen foods, providing up to three meals daily for a maximum of six months, delivered to the member's home or private residence, including shelters that do not offer meals. Nutrition Education involves evidence-based strategies for making healthier food choices and adopting nutrition-related behaviors, focusing on person-centered guidance that respects dietary preferences and cultural appropriateness. Pantry stocking offers a portion of recommended daily food for up to six months, with options including fresh, frozen, dried, pureed, or canned items, delivered as a grocery box or through a pre-loaded card for select grocery stores. A benefit for fresh fruits and vegetables is also available for up to six months, with similar delivery methods. The intake process includes calling, emailing, faxing, or visiting the website for more information, followed by a referral from the CCO to a registered dietitian for an assessment to verify the need for MTMs and ensure that care plan goals are met. There are no fees associated with this service.
Who can use this
- Members of CPCCO facing food insecurity can access HRSN nutrition services only once in their lifetime; they cannot receive MTMs and Nutrition Education simultaneously but can access them sequentially. Eligibility requires experiencing at least one specified life situation and one qualifying health condition, such as recent incarceration, homelessness, or being a young adult with a chronic health issue.
