Individuals who have experienced theft of their food benefits can file a claim to receive replacements, with a deadline of 30 days or by October 22, 2023.
Claim Submission Process: Claims may be filed in person at a Community Service Office, via phone through the Customer Service Contact Center, online through MyDocs, or by mailing or faxing a completed DSHS Claim of Stolen EBT Benefits form.
