About this resource
Nutrition Support is a service related to health-related social needs (HRSN) provided by the Oregon Health Plan, aimed at helping members whose health conditions can benefit from nutritional assistance. This service may include: Medically Tailored Meals (MTMs), which consist of fresh or frozen food, offering up to three meals daily for a duration of six months, delivered to the member's home or private residence, including shelters without meal provisions. Nutrition Education provides evidence-based strategies for making healthy food choices and adopting nutrition-related behaviors, focusing on person-centered, culturally appropriate dietary guidance. Pantry stocking offers a selection of recommended daily food for up to six months, which may include fresh, frozen, dried, pureed, or canned items, delivered as a grocery box or through a pre-loaded card for specific grocery stores. Additionally, a benefit for fruits and vegetables is available for up to six months, including various forms of produce, also delivered as a grocery box or via a pre-loaded card. The intake procedure involves contacting the service via phone, email, or website for more information. Following an initial screening, a referral is made to a registered dietitian to assess the need for MTMs and ensure that care plan goals are met. There are no fees associated with this service.
Who can use this
- Members of the Eastern Oregon CCO facing food insecurity can access HRSN nutrition services only once in their lifetime, with restrictions on simultaneous receipt of MTMs and Nutrition Education. Eligibility requires experiencing specific life situations and health conditions.
