About this resource
Offers support for mobility and medical equipment to low-income individuals, seniors, and families who have been referred by a professional. A minimum copayment of 25% from other funding sources is necessary.
Intake Procedure: Applications can only be initiated and submitted by a professional representative on behalf of the recipient, such as a social worker, patient navigator, case manager, clergy, counselor, or teacher. Applications can be requested through the website. Friends of Man pays vendors directly for approved items, and does not distribute funds to applicants. Note that vendors are not eligible to apply. Applications must be sent via email or U.S. mail, and fax submissions are not accepted.
Fees: There are no fees.
Who can use this
- Individuals with low income who have a professional referral to the program are eligible, with a required minimum copayment of 25% from other sources, and must be at or below 300% of the Federal Poverty Level.
