Nutrition Support is a service related to health-related social needs (HRSN) provided by the Oregon Health Plan. It aims to assist members with health-related issues where nutritional support can enhance health outcomes. This service may encompass: Medically Tailored Meals (MTMs), which can include fresh or frozen food, providing up to three meals daily for a maximum of six months, delivered to the member's home or private residence, including shelters without meal provisions. Nutrition Education offers evidence-based advice on making healthier food choices and adopting food-related behaviors that promote well-being, tailored to individual dietary preferences and cultural contexts. Pantry stocking supplies a portion of the recommended daily food intake for up to six months, with options for fresh, frozen, dried, pureed, or canned foods, delivered as a grocery box or via a pre-loaded card for specific grocery stores. The fruits and vegetables benefit lasts for six months and includes various forms of produce, also delivered as a grocery box or through a pre-loaded card for eligible members.

Intake Procedure: Interested individuals can call, email, fax, or visit the website for further details. Following an initial screening, a referral to a registered dietician is made by the CCO to assess the need and ensure that the goals of the MTM care plan are met.

Fees: There are no associated costs.

Who can use this

  • Members of Health Share facing food insecurity can access HRSN nutrition services only once in their lifetime, with restrictions on simultaneous receipt of MTMs and Nutrition Education. Eligibility requires experiencing specific life situations and qualifying health conditions.
