About this resource
Health-related social needs (HRSN) refer to fundamental requirements that impact an individual's health. This program offers qualifying members support for essential needs, such as assistance with rent and utility payments, tenant support, and storage fees, but services can only be accessed once during their lifetime.
Intake Procedure: For more information, members can call, email, fax, or visit the website; all necessary documents must be submitted for screening.
Fees: There are no associated fees.
Who can use this
- HealthShare members must meet several criteria: -Be 65 years or older, have children aged 0-5, be pregnant or have given birth in the last 12 months, or have a qualifying health condition -Have a household income at or below 30% of the average yearly income in their area -Require assistance to maintain housing -Possess a lease or written rental agreement; if not, they can complete a "verification of landlord-tenant relationship" form with their landlord to apply -Lack resources to avert homelessness.
Documents you may need
- -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
