About this resource

Nutrition Support is a health-related social need (HRSN) service provided by the Oregon Health Plan that assists members facing health-related issues where nutritional support can enhance health outcomes. This service may encompass: Medically Tailored Meals (MTMs), which include fresh or frozen food, offering up to three meals daily for a maximum of six months, delivered to the member's home or private residence, including shelters without meal provisions. Nutrition Education provides evidence-based strategies for making health-conscious food choices and guidance on culturally appropriate and person-centered dietary preferences. Pantry stocking supplies a portion of the recommended daily food intake for up to six months, with options for fresh, frozen, dried, pureed, or canned foods delivered as a grocery box or via a pre-loaded card for specific grocery stores. Fruits and vegetables are also included for up to six months, offering various forms like fresh, canned, frozen, or dried, delivered similarly.

Intake Procedure: Interested individuals can call, email, or visit the website for further information. Following an initial screening, a referral is made by the CCO to a registered dietitian for an assessment to validate the need for MTMs and ensure that the MTM care plan objectives are fulfilled.

Fees: There are no associated costs.

Who can use this

  • IHN members facing food insecurity can access HRSN nutrition services once in their lifetime, with restrictions on simultaneous receipt of MTMs and Nutrition Education. Eligibility requires experiencing specific life situations and qualifying health conditions.
