Nutrition Support is a service under the health-related social needs (HRSN) offered by the Oregon Health Plan, aimed at aiding members with health issues that can benefit from nutritional assistance. This service may encompass: Medically Tailored Meals (MTMs), which consist of fresh or frozen food, providing up to three meals daily for a maximum of six months, delivered to the member's home or private residence, including shelters without meal services. Nutrition Education provides evidence-based strategies for making healthier food choices and adopting beneficial food-related behaviors, ensuring that guidance is person-centered and culturally suitable. Pantry stocking offers a portion of the recommended daily food intake for up to six months, with options including fresh, frozen, dried, pureed, or canned goods, delivered as a grocery box or via a pre-loaded card for use at select grocery stores. The fruits and vegetables benefit lasts for up to six months, including various forms of produce, also delivered as a grocery box or pre-loaded card. The intake process involves contacting the service through calls, emails, faxes, or their website for further details. Following an initial screening, a referral is made to a registered dietician for an assessment to confirm the necessity for MTMs and to ensure the goals of the MTM care plan are achieved. There are no fees associated with this service.
