Intake Procedure: For further information and available services, members should call. Additionally, members can access the website to manage their personal health information, update their primary care provider, request a new ID card, and more.
Fees: There are no associated costs.
Who can use this
- Individuals enrolled in a Molina managed care plan. Residents of Washington state with incomes at or below 138% of the Federal Poverty Level may qualify for this plan if they do not qualify for WA Apple Health (Medicare). Additionally, children from families earning under 300% of the Federal Poverty Level may also be eligible.
