About this resource
This program offers a basic pair of eyeglasses with single or lined bifocal lenses through an e-voucher system, without covering the cost of eye exams. Approved applicants are required to place their orders through New Eyes' online eyeglasses dispensary.
Intake Procedure: Applications can be submitted online. While it is preferable for a social service agency or health advocate to apply on behalf of a client, individuals also have the option to self-refer.
Fees: There is a $15 non-refundable application fee to cover administrative expenses.
Who can use this
- Individuals with low income, living in households earning below 250% of the Federal Poverty Level (FPL), and who have had a recent eye exam (within the last 24 months) with a pupillary distance measurement.
