About this resource
Delivers ongoing, individualized support for individuals diagnosed with dementia and their caregivers. The Care Navigator creates a tailored care plan, facilitates access to necessary tools and resources, and conducts monthly check-ins. Medicare and private insurance are accepted.
Intake Process: Interested individuals can either call or fill out an online form. A scheduler will clarify the available services and assist in finding the most suitable option, as well as explain billing procedures and insurance coverage.
Fees: For clients whose primary insurance is Medicare, dementia care management support is provided at no charge. The program is in-network with several providers, including Regence (Blue Cross Blue Shield), Aetna, United Healthcare, Anthem, Humana, Medicare, and most Medicare Advantage Plans. A co-pay or deductible might be applicable.

- Residents of Washington state facing memory loss or significant aging-related conditions, along with their caregivers.
