About this resource
Health-related social needs (HRSN) refer to fundamental requirements that impact an individual's health. The benefits provided assist eligible members with essential needs such as rent and utility payment assistance, tenant support, and storage fees. Each member is entitled to receive HRSN housing services only once throughout their lifetime.
Intake Process: For more details, you can call, email, or visit the website; all necessary documents must be submitted for screening.
Fees: There are no fees involved.
Who can use this
- Members of PacificSource Community Solutions who qualify must meet the following criteria: -Be 65 years or older, have children aged 0-5, be pregnant or have given birth in the past year, or have a qualifying health condition -Have a household income at or below 30% of the average yearly income in their area -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack the resources to avert homelessness.
Documents you may need
- -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
