About this resource
Health-related social needs (HRSN) encompass fundamental requirements that influence a person's health. The program provides eligible members with support for basic necessities, such as assistance with rent and utility payments, tenant support, and storage fees. Importantly, members can access HRSN housing services only once during their lifetime.
Intake Procedure: For more information, individuals can call, email, or visit the website; all necessary documents must be submitted for screening.
Fees: There are no associated fees.
Who can use this
- Members of PacificSource Community Solutions must meet the following criteria: -Aged 65 or older, children aged 0-5, pregnant or have given birth within the last 12 months, or have a qualifying health condition -Household income at or below 30% of the average yearly income for their area -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack resources to avert homelessness.
Documents you may need
- -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
