About this resource
Nutrition Support is a health-related social need (HRSN) service available through the Oregon Health Plan, aimed at assisting members with health conditions that could be improved through nutritional support. The service may encompass: Medically Tailored Meals (MTMs): -These meals can include fresh or frozen options -Members can receive up to three meals daily for a maximum of six months -Meals are delivered to the member's home or private residence, which can include shelters without meal provisions. Nutrition Education: -This includes evidence-based guidance on making healthier food choices and adopting nutrition-related behaviors that promote well-being, tailored to individual preferences and cultural considerations. Pantry stocking: -Provides a portion of the recommended daily food intake for up to six months, offering fresh, frozen, dried, pureed, or canned options. Eligible members will receive this benefit as a grocery box or a pre-loaded card for use at participating grocery stores. Fruits and vegetables: -Available for up to six months, this benefit includes fresh, canned, packaged, frozen, dried fruits and vegetables, and fresh herbs, delivered as a grocery box or through a pre-loaded card for grocery store use.
Intake Procedure: Interested individuals can call, email, or visit the website for further details. Following an initial screening, a referral is made by the CCO to a registered dietitian for an assessment to confirm the need for MTMs and to ensure that the care plan objectives are achieved.
Fees: There are no associated costs.
Who can use this
- Members of PacificSource Community Solutions facing food insecurity are eligible for HRSN nutrition services only once in their lifetime and cannot receive MTMs and Nutrition Education simultaneously, although they can access Nutrition Education before or after MTMs. Eligibility also requires experiencing at least one specified life situation and one qualifying health condition.
