The Coordinated Housing Access Team (CHAT) is a collaborative effort among agencies aimed at aiding homeless individuals and adult family units in finding resources related to permanent supportive housing. The CHAT team utilizes the Multnomah Services and Screening Tool (MSST) assessment (previously known as SPDAT) to evaluate the risk levels of individuals or family units and prioritize the services they require. During the assessment, information regarding shelter and transitional housing will be shared, although immediate services are not provided.

Intake Procedure: To begin the process, visit the website and complete the Permanent Supportive Housing Pre-Screening Form for the assessment.

Fees: There are no fees involved.

Who can use this

  • To qualify, households must include at least one adult aged 18 or older without minor dependents and meet one of the following conditions: -Currently in an emergency shelter, transitional housing, or an unsuitable living situation (e.g., car, streets, tent, abandoned building) -In an institution (e.g., jail, hospital, treatment facility) and at risk of homelessness upon release -Doubled up with family/friends without a lease and at risk of literal homelessness within 14 days -Currently housed but facing imminent homelessness within 14 days or have received an eviction notice -Additional eligibility criteria may be applicable.
