About this resource
Health-related social needs (HRSN) are essential requirements impacting an individual's health. Benefits are provided to eligible members to assist with fundamental needs such as rent and utility payment help, tenant support, and storage costs.
Intake Procedure: For more details, you can call, email, fax, or visit the website; all necessary documents must be submitted for screening.
Fees: There are no fees.
Who can use this
- Eligible Trillium members include those who are 65 or older, children aged 0-5, pregnant or who have given birth in the past year, or anyone with a qualifying health condition, with a household income at or below 30% of the local average, needing housing support, having a lease or rental agreement, lacking resources to avoid homelessness, and can only access HRSN housing services once in their lifetime.

- Medicaid ID number, Rental/lease agreement, or utility bill
