Nutrition Support is a service addressing health-related social needs (HRSN) offered by the Oregon Health Plan. It assists members with health conditions that can benefit from nutritional support, potentially enhancing their health outcomes. The program may provide: Medically Tailored Meals (MTMs), which include fresh or frozen meals delivered to members' homes for up to three meals daily over a six-month period, including options for those in shelters without meal provisions. Nutrition Education offers evidence-based guidance on healthy food choices and culturally appropriate dietary practices. Pantry stocking supplies a portion of the recommended daily food intake for up to six months, with options for fresh, frozen, dried, pureed, or canned foods, delivered as grocery boxes or via pre-loaded cards for grocery store use. Additionally, members can receive fresh fruits and vegetables for up to six months through similar delivery methods.
