About this resource
Health-related social needs (HRSN) are essential requirements impacting an individual’s health. These benefits are designed to assist qualifying members with fundamental needs, such as assistance with rent and utility payments, tenant support, and storage fees.
Intake Procedure: For more information, you can call, email, fax, or visit the website; all necessary documents must be submitted for screening.
Fees: There are no fees involved.
Who can use this
- Eligible Trillium members must meet the following criteria: -Aged 65 or older, have children aged 0-5, be pregnant or have given birth within the last year, or have a qualifying health condition -Household income at or below 30% of the average annual income for their area -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack resources to avoid homelessness -HRSN housing services are limited to one use per member's lifetime.

- Medicaid ID number, Rental/lease agreement, or utility bill
