About this resource
Nutrition Support is a service related to health-related social needs (HRSN) available through the Oregon Health Plan. This program aids members with health conditions that can benefit from nutritional support, which may include: Medically Tailored Meals (MTMs) that consist of fresh or frozen foods, offering up to three meals daily for a maximum of six months, delivered directly to the member's home or private residence, including shelters without meal provisions. Nutrition Education provides evidence-based guidance on making healthier food choices and adopting beneficial food-related behaviors, tailored to individual dietary preferences and cultural considerations. Pantry stocking offers a portion of the recommended daily food intake for up to six months, with options for fresh, frozen, dried, pureed, or canned foods, delivered as a grocery box or via a pre-loaded card for use at specific grocery stores. Additionally, members can receive a benefit for fresh, canned, packaged, frozen, or dried fruits and vegetables for up to six months, also delivered or accessible through a pre-loaded card. The intake process involves contacting the service via phone, email, or website for further information. Following an initial screening, a referral is made to a registered dietitian for an assessment to confirm the need for MTMs and to align with care plan objectives. There are no associated fees.
Who can use this
- Members of Umqua Health Alliance facing food insecurity may access HRSN nutrition services once in their lifetime, with restrictions on concurrent receipt of MTMs and Nutrition Education, and must meet specific life situations and health conditions.
