About this resource
Health-related social needs (HRSN) refer to essential requirements that influence an individual's health. These benefits assist qualifying members with necessities such as rent and utility payment support, tenant assistance, and storage fees. Each member is entitled to receive HRSN housing services only once throughout their lifetime.

Fees: There are no fees associated.
Who can use this
- Members of Umpqua Health Alliance must meet the following criteria: -Be 65 years or older, have children aged 0-5, be pregnant or have given birth within the last 12 months, or have a qualifying health condition -Have a household income at or below 30% of the average yearly income in their area -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack resources to avert homelessness.
