About this resource
Nutrition Support is a service related to health-related social needs (HRSN) available through the Oregon Health Plan. It aids members facing health-related issues where nutritional assistance can enhance health outcomes. The service may encompass: Medically Tailored Meals (MTMs), which can include fresh or frozen foods, providing up to three meals daily for a maximum of six months, delivered to the member's home or private residence, including shelters without meal provisions. Nutrition Education offers evidence-based strategies for making healthier food choices and adopting nutrition-related behaviors that promote well-being, tailored to individual dietary preferences and cultural considerations. Pantry stocking provides a portion of the recommended daily food intake for up to six months, including fresh, frozen, dried, pureed, or canned items, delivered as a grocery box or through a pre-loaded card for specific grocery stores. Additionally, members can receive benefits for fresh, canned, packaged, frozen, or dried fruits and vegetables for up to six months, similarly delivered or accessed via a pre-loaded card.
For intake, individuals can call, email, or visit the website for further details. Members are required to consult a registered dietician for an assessment to validate the necessity for MTMs and to ensure that the MTM care plan objectives are achieved. YCCO members do not require prior authorization to see a dietician but may consider obtaining a referral from their primary care provider.
There are no associated fees.
Who can use this
- Members of Yamhill Community Care facing food insecurity can access HRSN nutrition services only once in their lifetime and cannot receive MTMs and Nutrition Education simultaneously, although they may do so sequentially. They must also be experiencing specific life situations and qualifying health conditions.
