About this resource
Individuals with disabilities are provided with tailored care in a group home environment. They are supported in making personal choices regarding their daily activities, roommates, staff members, and personal goals. The program offers up to 24 hours of personal assistance for those who can maintain a degree of independence.

- A documented disability is required; this includes adults with developmental disabilities, mental health diagnoses, and older adults.
