About this resource
This initiative assists frail adults in staying in their homes by offering and coordinating multiple services. A social worker evaluates each client's needs and formulates a care plan to meet those needs. Services provided encompass grocery shopping and other errands.

- Serves individuals aged 60 and above; medically fragile adults over 60 who comply with income requirements.
