- Eligible participants include females who are pregnant or breastfeeding, those who have recently given birth, or children under 5 years old residing in Weber or Morgan counties.
Documents you may need
- Photo ID — Valid driver's license, Medicaid card, birth certificate, passport, work or school ID, state-issued identification card,; Income Verification — All that apply: Pay statements for the last 30 days, Medicaid card, proof of Family Employment Program (TANF), food stamp eligibility notice, child support, self-employment records; Proof of Address — Current bill or rental agreement with name and address
