This program offers group home or host home living arrangements for individuals needing additional supervision and assistance with developmental and community living skills.
Who can use this
- Applicants must have a documented disability and fall into one of three categories: intellectual disabilities or related developmental conditions (such as autism, Down syndrome, or genetic disorders), Acquired Brain Injury (ABI), or physical disabilities that result in the functional loss of at least two limbs. Additionally, they must be residents of Utah and U.S. citizens or legal permanent residents, and possess a Medicaid Waiver.
