Who can use this
- Eligible participants include those who are homeless or have an income below 200% of the Federal Poverty Level, and they must obtain a referral from a physician associated with the program.

- Proof of Insurance — Insurance card if applicable; Income Verification — Paystubs, W-2, if applicable.; Photo ID — Photo ID, if available not required.
