About this resource
Diabetes Self-Management Education & Support (DSMES) assists individuals with diabetes in making informed choices, collaborating with their healthcare teams for necessary support, and improving their hemoglobin A1C, blood pressure, and cholesterol levels. This program teaches participants how to maintain a healthy diet, boost physical activity, monitor their blood sugar, manage medications, handle the emotional aspects of diabetes, and lower the risk of additional health issues.
Who can use this
- Participants must be at least 18 years old and have a body mass index (BMI) of 24 or higher.
