Care coordinators assist families in connecting to early childhood resources throughout Utah. Based on the specific needs of the family, they might offer contact details for resources or make calls to schedule appointments on their behalf. After making referrals, care coordinators will check in with the family after one month and again after three months, with the possibility of further follow-ups. Additionally, they provide a searchable database of early childhood resources.
