About this resource
Guardianship services provide support for individuals lacking available resources or those at risk of abuse, neglect, or exploitation.
Who can use this
- To qualify, individuals must be at least 18 years old, residents of Utah, without family members willing to act as guardians, unable to afford a private guardian, and assessed as having an incapacity in processing information, making decisions, or meeting basic needs.

- Online referral form.; Medical statement and/or psychiatric assessment.
