Who can use this
- Applicants must have a documented disability; to be eligible for equipment, they should have a hearing or speech impairment and either be part of a public assistance program managed by a state agency or have an income at or below 200% of the 2024 federal poverty level. Public assistance programs may include Medicaid, CHIP, Lifeline/Utah Telephone Assistance Program (UTAP), SNAP, or HEAT.
Documents you may need
- Income Verification — Income documentation can include: tax returns, a Social Security benefit letter, a Social Security disability benefits letter (SSDI, SSD); The application must be certified by one of the following healthcare providers: a licensed physician, licensed physician assistant, otolaryngologist, audiologist, speech-language pathologist, or a qualified state agency employee.
