This initiative provides nutrition education, vouchers for healthy foods, breastfeeding assistance, reviews of immunization records, and connections to additional support services for women, infants, and children under the age of five.
Application Process: Submit your application online at: https://saltlakecounty.gov/health/wic/
Who can use this
- Eligibility includes females who are pregnant or breastfeeding, recent mothers, or children under five, with income criteria to be met.
Documents you may need
- Photo ID — Valid proof of ID for the individual applying for WIC and all benefitting children.; Income Verification; Immunization record for each child.; Proof of Address
