Who can use this
- Children in Utah who are 3 or 4 years old by September 1st are eligible, with priority given to families with children with disabilities or those whose income falls within the Federal Poverty Guidelines.

- Birth Certificate; Proof of Address; Income Verification; Application, nutrition, and health history.
