About this resource
The program offers participants services such as personalized nutritional counseling from a registered dietitian or nurse, access to healthy foods and formulas, support for prenatal and breastfeeding needs, and connections to additional resources.
Intake Procedure: Applications can be submitted online at: https://wic.utah.gov/locations/
Who can use this
- Eligible participants include females who are pregnant, breastfeeding, or postpartum, as well as infants and children under 5 years old, provided they meet income criteria and are residents of the state. Additionally, those already receiving SNAP, TANF, or Medicaid automatically qualify.
Documents you may need
- Proof of Address — Current utility bill with a street address or a rent or mortgage receipt.; Income Verification — One of the following: (1) Pay statements showing income for the last 30 days for each household member who is employed, (2) Medicaid card, and a letter from Workforce Services showing amounts received for SNAP and TANF.; Photo ID — Driver's license, Medicaid card, birth certificate, or passport for each family member. Crib card or hospital document can be used for infants under 1.
