This service issues identification cards and provides support for renewals, modifications, and reporting lost or stolen IDs for existing cardholders.
Intake Procedure: Applications can be submitted online at: https://dld.utah.gov/id-sort/
Who can use this
- Eligible individuals include U.S. citizens or those with proof of lawful presence, with parents required to sign for applicants under 16.
Documents you may need
- Social Security Card — SSN card or proof of ineligibility to obtain a social security number.; Proof of Address — 2 forms of proof for your Utah address.; Photo ID — Proof of identity including name and date of birth.
