About this resource
This initiative supports low-income women, infants, and children under the age of five by providing nutrition education, vouchers for healthy foods, breastfeeding assistance, immunization record assessments, and connections to other supportive agencies.
Intake Procedure: Applications can be submitted online at: https://saltlakecounty.gov/health/wic/
Who can use this
- Eligible participants include females who are pregnant or breastfeeding, women who have recently given birth, or children under five, with income criteria that must be met.
Documents you may need
- Income Verification — Proof of income for the household.; Photo ID — Valid proof of ID for the individual applying for WIC and all benefitting children.; Immunization record for each child or infant.; Proof of Address
